WHAT IS BREAST AUGMENTATION & WHEN IS IT RECOMMENDED?
Breast augmentation is a common plastic surgery procedure that increases the size and changes the shape of breasts. Dr. Maloney can surgically enhance your breasts using a silicone or saline breast implant. Placement of an implant can successfully improve how your breasts look and feel. Some of the most common reasons a patient may choose a breast augmentation would be:

Implant Options
There are several different options of implants in the market today. When you come in for a consultation, Dr. Maloney will discuss the options that will work best for you and your goals.
Saline implants
Saline implants are composed of a silicone shell filled with sterile saline. These implants are empty when Dr. Maloney places them in the pocket. They are then filled to the appropriate volume. These implants are most popular in women who are looking for an overfilled, round appearance to their breasts.
Silicone implants
Silicone implants are composed of a silicone shell, filled with a cohesive silicone gel. The gel implants are the most popular implants in the market today. Unlike their predecessors that were a silicone shell, filled with a silicone oil; the new generation of highly cohesive gel implants can literally be cut in half and the gel will remain in place. Placement of the cohesive gel implants gives you a soft, natural look and feel to the breasts.
Determining Implant Size
Many patients ask about the different profiles of implants and when each should be used. The most important measurement we take at your consultation is the measurement of the base diameter or width of your breasts.
Once Dr. Maloney has taken this measurement and discussed your goals, he can offer you some choices of implants that would fit well into that measurement.
- If you choose an implant that is wider than the base diameter, the implants may be too wide for your body and be “in your armpit”.
- If you choose something too narrow, the implants do not fill out the entire breast. The low, moderate, moderate-plus, high and ultra-high implants allow Dr. Maloney to choose a variety of implant volumes with you, but still remain in the same base diameter. For example if your base diameter is 12cm, the volume in the moderate implant is 235cc, the moderate-plus is 300cc, the high profile is 375cc and the ultra-high profile is 535cc.
As you can see, this gives both you and Dr. Maloney several options in terms of size and volume while remaining within the same base diameter measurement.
Implant Placement
There are two common options when deciding where to place the implant. These are the submuscular plane, referred to as “under the muscle” and then the sub-glandular plane, referred to as “on top of the muscle”. The common misconception is “under the muscle” is completely under the muscle. This is not the case. It is actually a dual-plane approach where the upper portion of the implant is covered by the muscle and the bottom portion is not. In the past, it was thought that the submuscular placement would decrease the chance of a capsular contracture.
Over the years, this has been shown not to be the case. Patients who are best for submuscular placement typically have little to no breast tissue and need more tissue coverage over the implant. The downside with the submuscular placement is when you flex the chest muscles, the implants are pulled laterally, which some patients dislike. This is called an animation deformity. Many patients are choosing the sub-glandular placement to avoid the animation deformity and speed up the recovery since the muscle is not divided. Someone with a good amount of breast tissue and quality skin, is typically and excellent candidate for this option.

Scheduling a consultation for plastic surgery
When you meet with Dr. Maloney and his team for a consultation, we will first evaluate your appearance and discuss your perception of issues regarding your appearance, as well as your needs and goals. We will take 3D images of your body from your belly button to your neck and create a rendering of what you may look like with various sizes of breast implants.
During the consultation, Dr. Maloney will determine if you are a good candidate for a breast augmentation. He may also recommend other plastic surgery procedures in addition to or instead of breast augmentation such as a breast lift or mastopexy.

Preparing for a breast augmentation
Your procedure will be performed at an outpatient surgery center. Our team will go over the surgical fees, scheduling dates, and financing options. We will look at your 3D images to make a final decision about breast implant style, size, and shape. Here’s a look at your pre- and post-breast augmentation schedule:
Three weeks pre-plastic surgery
If you smoke, you need to stop smoking. You should also stop taking aspirin and other blood thinning medications. You will come in for another consultation with our team, and you may be given prescriptions to fill before the plastic surgery procedure. We will need to review any medications, supplements, and vitamins you’re currently taking.
One week pre-plastic surgery
Stop taking diet pills and/or Metformin. Purchase a sports bra that zips up the front, and ice packs. You should begin taking an over the counter stool softener. Make sure you have picked up all prescribed medications from the pharmacy.
24 hours before surgery
If your plastic surgery is before 10am, don’t eat or drink anything after midnight, including water. Get a good night’s sleep.
Day of surgery
Make sure you have a reliable ride to and from the procedure. If your plastic surgery is after 10am, drink 16-24 ounces of Gatorade four hours before the procedure is scheduled. Take any prescribed antibiotics the morning of surgery with a small sip of water to ensure the antibiotics are in your system before your procedure. Do not wear lotions, makeup, deodorant, jewelry, or nail polish. Do not chew gum or breath mints the morning of surgery. Wear comfortable clothing that is easy to take off and put on.
Dr. Maloney will meet you at the surgery center and confirm the implant size and answer any remaining questions. He will also confirm your cellular phone number and send you a text so you have his personal cell phone number which you may use for any questions or concerns after hours. He will also send you a text or call you from his personal cellular phone the night of surgery to check on you.
24 hours post-plastic surgery
Someone will need to stay with you for the first 24 hours post-surgery. After 24 hours have passed, you can remove your dressings and shower. One to three days after surgery, you will come back in for a post-op exam. The team at Maloney Plastic Surgery is available 24 hours a day for any questions or concerns that may come up during your recovery.
One to three weeks post-plastic surgery
Continue taking your antibiotics as prescribed until they are gone. Wear a sports bra or snug camisole for the first three weeks. You can go shopping for new bras about 3 weeks post surgery. Your sutures are dissolvable, however if there is still a small knot at 3 weeks, the knot will be removed. The steri-strip covering the incision will fall off within the first week. You will need to take a couple days to a couple weeks off of work depending on the type of work you do. People with a “desk-job” will typically be back to work within 3-7 days. We release you to the gym 3 weeks after your procedure at about 50% of your “normal” routine.
