What is Bioidentical Hormone Replacement Therapy?

Bioidentical hormone replacement (BHRT) is a treatment that uses hormones that are chemically identical to the ones the human body produces. BHRT is used to treat symptoms associated with hormonal imbalance and hormone deficiencies. An imbalance or deficiency of hormones could be a natural result of the aging process, or as a symptom of an underlying medical condition or disease.

What are the Different Types of Hormone Replacement?

Hormone replacement therapy is available in different forms and can be administered using different methods, including pills, patches, and gels. The types of BHRT we offer are:

  • Estrogen-Only BHRT – Used for women who have had their ovaries removed or have an underlying condition that causes insufficient estrogen production, this treatment only contains Estrogen. It can be administered as a pellet, pill, patch, or vaginal suppository or cream. Estrogen only replacement is appropriate only for women who have had hysterectomies. 
  • Combined BHRT – Used for women who have a uterus, this treatment contains estrogen and progesterone.  Progesterone is typically administered as a pill. There are two types of combined BHRT: sequential combined HRT, during which you take estrogen and progestogen in a cyclical pattern; and continuous combined HRT, during which you take estrogen and progesterone daily.  Your stage of life and current menstrual status will determine which therapy option is best for you.
  • Testosterone Replacement Therapy – Used to address low libido and other symptoms of low testosterone, this hormone only contains bioidentical testosterone and is administered as a troche, cream, injection or pellet.

What are the Benefits and Risks?

The benefits of BHRT are relief from symptoms, improved sleep, improved focus and mental clarity, improved mood, better bone health and bone density, improved cardiovascular health, and improved quality of life. Like any other medical treatment, BHRT also has a chance of negative side effects or risks. Potential side effects of hormone replacement include, but are not limited to: abnormal uterine bleeding, growth of estrogen-dependent tumors, hypersexuality, acne, increased body/facial hair growth, hair loss or thinning.

What to Expect at Your Consultation

We take a highly individualized approach to BHRT. At your consultation, we will take your full medical history, current medication use and any past hormone replacement therapy. We will discuss your symptoms and how they are affecting your life. We will explain the benefits and risks of BHRT so that you can make an informed decision about whether this treatment is right for you. You will then be given a lab order to obtain blood to determine your hormone levels. Your results should be ready in one or two weeks. You will then come in for another consultation to go over your lab results and undergo a SECA body composition scan. We can use your results to help you gain a deeper understanding of your overall health and symptoms and the underlying cause of your hormone imbalance or deficiency. When we have a full picture of your health and symptoms, we can design a hormone replacement therapy program that meets your needs and restores your energy, mood, and overall health and quality of life with minimal side effects.

Dr. Maloney is a Harvard trained, board certified plastic surgeon who returned to his home town of Tucson, AZ to open Maloney Plastic Surgery in 2002.

How is Hormone Replacement Therapy Administered?

Most types of hormone therapy can be administered in multiple ways: 

  • Orally in the form of troches, tablets or capsules
  • Transdermal in the form of patches, gels, creams, or sprays
  • Vaginally in the form of creams
  • Intramuscular injections 
  • Implantable in the form of a pellet that is implanted under the skin

 

We typically recommend implantable hormone pellets, as they deliver optimal doses of hormones gradually over time, allowing you to achieve more natural results with fewer side effects. We will need to replace your pellet every three to five months.

The first part is the virtual consultation where you will fill out some basic information such as the procedure you are interested in having performed, and you will upload a front and side view photo. Dr. Maloney will review these and determine if you are potentially a surgical candidate as well as give you pricing.
