How to choose your plastic surgeon
This form has a list of questions you should ask your plastic
surgeon. It is available for
you to print out and bring with you to your appointment with
Dr. Maloney as well as
compare it to any other plastic surgeons you have consulted with.
Patient History Form
This is the history form we will ask you to fill out on your
first visit to our office. Knowing your
time is valuable, we have this form available to print out prior
to your appointment so you can
fill it our in the comfort of your own house and bring it with
